New Patient Information

If you are visiting our practice for the first time, there are a few things you can do to make your visit more streamlined. You can fill out your new patient forms at home and send them to us or print them out and bring them with you. If you don’t want to fill them out at home, please arrive 10-15 minutes early to fill out the forms before your appointment. If you have insurance, please bring your insurance card with you. If you have had X-rays taken within the last six months, you can have your previous practice send them to our office if you would like.

Your Financing Options

Whether you have dental insurance or not, Vista Ridge Family Dentistry would be happy to work with your family! In addition to accepting cash, debit and credit cards, and FSAs and HSAs, we also offer an in-house membership plan that helps uninsured patients save money on routine preventive care and other services. We also accept third-party payment plans through both GreenSky® and CareCredit®, which can help make dental care more budget-friendly. For an up-to-date list of providers that we’re in-network with, please contact our office.

We take the time to get to know each patient to better understand them and their oral health needs. Our team wants to know about your smile goals, any questions or concerns you may have, and more about your dental history, so we can better serve you and your smile.
