New Patient Information
If you are visiting our practice for the first time, there are a few things you can do to make your visit more streamlined. You can fill out your new patient forms at home and send them to us or print them and bring them. If you don’t want to fill them out at home, please arrive 10-15 minutes early to complete the forms before your appointment.
If you have insurance, please bring your insurance card with you. Finally, if you have had X-rays taken within the last six months, you can have your previous practice send them to our office if possible.

Whether you have dental insurance or not, Vista Ridge Family Dentistry will work with you to help you find a suitable payment option for your oral health care. In addition to accepting cash, debit, credit cards, FSAs, and HSAs, we also offer an in-house membership plan that helps uninsured patients save money on routine preventive care and other services.
We also accept third-party payment plans through both GreenSky® and CareCredit®, which can help make dental care more budget-friendly. Please contact our office for an up-to-date list of providers we’re in-network with.
